No entry fee for general entries. $50.00 entry fee for political only. Donations are welcome! 1st place prize will be awarded for each category below. Monday, August 4, 2008 is the deadline for applications to be judged. Non-judged entries accepted until 10:30 AM on Parade Day.

Judges will give extra consideration for entries following the theme “Jewel of the Valley”.

PROOF OF INSURANCE:

Verification of insurance is required for equestrian and ALL motorized or wheeled vehicles.

Important: In order for the parade announcer to give your entry the best possible publicity, it is important that you fill out this form with as much detail as possible and return it with your entry fee (if applicable), insurance and/or Indemnity Agreement to: Cascade Days Parade, PO Box 251, Concrete, WA 98237

All entries must be received by Monday, August 4, 2008 to be eligible to be judged.

All others may enter, but won’t be eligible for prizes.

SAFETY ALERT!

v No Alcoholic beverages are to be consumed or carried on parade entries.

v Entries must observe all applicable rules and regulations and follow all instructions by parade officials and law enforcement.

v Candy or other materials may be HANDED out only as your entry proceeds down the parade route.

v In case of emergency, which may require ambulance or fire to leave parade, all entries are required to move as far to the right as possible.

v All motorized vehicles must proceed at a SLOW PACE. No speeding up, skidding, sudden breaking and “wheelies” will be allowed.

v All firearms must be unloaded and in lock position. For everyone’s safety!

HOLD HARMLESS:

I attest and verify that I have full knowledge of the risk in the Concrete Cascade Days Parade and I assume those risks and the possible medical and/or emergency expenses in the event of accident, illness or other incapacity, regardless of whether I have authorized such expenses. In consideration of the acceptance of my application, I/we agree to indemnify and hold harmless the Cascade Days Parade Committee, The Town of Concrete, Concrete Chamber of Commerce, andy and all sponsors, organizers or volunteers; against all claims resulting from our participation in the Concrete Cascade Days Parade. I also understand that the sponsors may, for publication or promotion, use my name or picture of my participating in this event without obligation or liability to me.
